My 16 year old niece is on Humalog and Lantus which allowed more freedom in her diet, but she has had terrible control along with several instances of hypoglycemia. For the last six months, she has been on a stricter diet, but her A1c still runs 9-10%, and her blood sugars are all over the place. Is it acceptable for teenagers to be allowed to run this high?

I am currently Glucotrol XL (twice daily) along with 100 units of Lantus every night, and three shots of Regular (25-30 units each dose). The problem is I am very sleepy during the day, I feel like I’ve been drugged, and I had to quit my job because of this about five weeks ago. I asked my doctor if I could take the Lantus in the morning, but he said it was better to take it at night.

I have had type 1 diabetes for 11 years, and my blood sugars run in the 500 mg/dl [27.8 mmol/L] and higher range constantly even though my doctors and I have tried everything from injections, to an insulin pump, and even two Islet cell transplants. I only weigh about 122 pounds, I take over 180 units of insulin per day, and my blood sugars never come down.

My 18 year old daughter, who has had insulin-treated type 2 diabetes for 10 years, has become more insulin resistant recently. I was warned four years ago that this was a highly likely scenario considering her other health problems, but I am concerned about the effects of this resistance and insensitivity. What are the ramifications of a body that no longer can process the insulin being injected?

I recently heard that an insulin spray for diabetes would be available to South Africa by year end which I find hard to believe as we are so far behind the Western world that we are not even using the pump yet. Has the spray been sampled and approved? When will it be available, if at all?

The doctor diagnosed my son with insulin resistance and dysmetabolic syndrome. He has prescribed a low-carb, high protein diet along with a half-hour of daily exercise as a first level of treatment. If that doesn’t appear to be working, he’ll move on to possibly treating with medication (Glucophage). I have several questions.

My daughter has had diabetes for 10 years, we just found out she is insulin resistant, and her doctors are not really sure what we can expect other than increased insulin. She also has a number of other problems, and her kidneys are already damaged. Where can I find information on insulin resistance? What can I expect to see happen? What can I do?

My seven year old boy, who was diagnosed with type 1 diabetes when he was two, is very active, and although I try very hard to keep his sugars pretty much controlled, sometimes I just cannot get his sugars to stay in the normal range. Is there anything I could do or try when this situation arises?
